Ascent Skilled Nursing Facilities Assure Breach Victims of “Credible Evidence” Stolen Data Was Deleted
On July 31, 2026, three skilled nursing facilities operated by Ascent (Bear Mountain Health and Rehabilitation, Elevate Health and Rehabilitation, and Swannanoa Valley Health and Rehabilitation) notified residents of unauthorized threat actor access to their systems.
